The format promises a comparison. The comparison it delivers is between two photographs of the outside of a person, taken at two moments they chose.

BPC-157 before and after is a photography format applied to a question photography cannot answer. The tissue in question sits under the skin. Tendon, ligament, muscle fibre and gut lining are not visible from outside, and the animal work that generated the evidence read them by removing them.

So the honest version of this topic is not a gallery. It is a specification: what a record would have to contain before the comparison meant anything.

The photograph problem

A photograph records surfaces. Swelling, bruising, posture, the angle of a shoulder. Those are real things and they do change during recovery.

They are also the things that change on their own. Bruising resolves. Swelling subsides. Posture shifts with pain, and pain shifts with time. A pair of photographs taken weeks apart during any recovery will differ, whatever was or was not taken in between.

What a photograph cannot show is collagen organisation in a healing tendon. That was measured in rats under a microscope. There is no camera version of it.

Video is a small step up and a large step short. A clip of a shoulder rotating further than it did before records something functional rather than cosmetic, which is genuinely more informative. It still has no baseline taken before the decision, no control, and no blinded assessor, and the person filming knows exactly which take to keep. Better instrumentation does not fix a missing comparison.

Stage one, the baseline that usually does not exist

A before image is only a baseline if it was taken as one. Most are not. They are taken after a decision has already been made, often after the worst point has passed, which is precisely when improvement is most likely regardless.

The animal studies weigh the rat and create the injury at a recorded moment. That is what a baseline is. It exists before the intervention and it was measured, not remembered.

Stage two, the injury clock nobody starts on time

Timing is central to this literature. In the rat tendon and muscle work, administration typically begins at a defined interval after an injury the researchers created, often very soon after.

Outside a laboratory, the clock is fuzzy. The injury happened at some point. It was noticed later. It was tolerated for a while. Something was tried. Then something else. By the time a before photo exists, weeks of unrecorded healing have usually passed.

A timeline built on an uncertain start date cannot support a claim about how fast anything worked.

Stage three, everything else that changed at the same time

Recovery is rarely a single variable. People rest more. They start physiotherapy. They reduce the load that caused the problem. They sleep differently. They often use several supplements or compounds at once.

Every one of those has a plausible claim on the result. A personal before and after has no mechanism for separating them, because separation requires holding things constant, and nobody lives that way.

Stage four, the control that cannot exist in one person

This is the structural one. The rat studies compare treated animals against untreated animals with the same injury. The difference between the two groups is the finding.

An individual has one injury and one course of healing. There is no untreated version of the same person running alongside. Untreated animals in these studies also improve, because tissue repairs itself, which is exactly why the comparison is the point and the treated group alone is not.

Stage five, the assessor who already knows

In a well designed study, the person scoring the outcome does not know which group the sample came from. That is not a formality. Expectation changes scoring, reliably, in every field where it has been examined.

A person judging their own shoulder knows what they took, what they paid, and what they hoped for. So does anyone reading the post. Nobody in the chain is blind, and every part of the assessment is exposed to that.

What one person can legitimately record

None of the above means a personal record is pointless. It means the record has to be honest about what it is, and a few choices raise its quality considerably.

Dated entries written at the time beat anything reconstructed later. Memory reorganises a recovery into a story with a turning point, and the turning point usually lands wherever the explanation is most satisfying.

Objective measures beat impressions. Range of motion measured the same way each time, load tolerated on a specific movement, or a count of interrupted nights are all things that can be compared across weeks. How the shoulder felt today is not, because the scale moves.

Recording everything else that changed is the part most logs skip and the part that matters most. Rest, rehabilitation sessions, training load, other substances, sleep, workload. A log that lists only the compound has quietly assigned the result to it before any analysis happened.

And the honest label on the finished record is a description, not a result. It documents one person's recovery alongside a set of things they did. It cannot say which of those things caused what, and no amount of care in the recording changes that, because the missing element is a comparison group rather than better data collection.

Where credible timelines will eventually come from

Human trials on BPC-157 are registered, which is why the claim that no human work exists is inaccurate. NCT07803250 is a Phase 1 study with a planned enrolment of 30 participants in recovery after rotator cuff repair, listed as not yet recruiting. NCT02637284 is a Phase 1 study of 42 participants covering safety and pharmacokinetics, with status listed as unknown.

None has published efficacy results. When they do, the reason those results will mean something is that the design supplies every row of the table above. A known injury date. A measured baseline. A control arm. Blinded assessment. Endpoints chosen in advance rather than noticed afterwards.

That is what a real before and after looks like. It is a dataset, not a pair of images, and it does not exist yet for this compound.

What the format is good for, and what it is not

None of this means personal accounts are worthless. They describe how people actually use things, what they expected, and what happened next in their own words. That is genuine information about behaviour.

It is not information about causation. The distinction is the entire subject, and blurring it is what makes the format misleading rather than merely limited.

There is also a supply side issue that no image can address. BPC-157 is sold as a research chemical with no marketing authorisation from the FDA, the EMA or the MHRA. Nobody photographing a shoulder knows what was in the vial, because the contents are unverified unless independently tested, and a photograph cannot test anything.
