Note the right-hand column. A single discounted vial at $67.00 does not come close to the $250 free-shipping threshold, so the first vial always carries carriage. That is worth factoring in when comparing a one-off purchase against a supplier with a lower threshold.
Those tier prices are reductions against the current price rather than the discounted price. Whether a tier and the code combine is worth reading off the cart total rather than assuming, because the answer decides whether buying multiples is worthwhile at all.
Why the per-milligram figure moves
Two laboratories have certified this product on two different batches, and they disagree about how much peptide is in the vial.

A spread of roughly 37% on the number most pages quote as a single confident figure. It runs in the buyer's favour on one batch and against them on the other, which is worth stating plainly rather than framing purely as a warning.
Neither certificate is defective. Purity and net content are separate measurements and both purity results were excellent, above 99% in each case. Quantity is what varied, and quantity is the measurement almost nobody in this market publishes.
Why insurance never enters into it
A cost article for an approved medicine would compare copay against cash price and weigh manufacturer savings programmes. None of that framework applies.
FOXO4-DRI holds no marketing authorisation from the FDA, the EMA or the MHRA. No approved product means no formulary listing, no negotiated rate, no savings card and nothing for an insurer to reimburse. It has also never been given to a human in a registered trial, so the evidence that normally precedes those arrangements does not exist either.
That leaves research-supply pricing as the only pricing there is.
Judging whether a price is plausible
Without a reference price to anchor against, the test is not whether a price is low but whether the documentation justifies it.
A vial well below market, sold with no lot number, no named laboratory and no measured content figure, is not a bargain. The cost of discovering it was not what the label said is unrecoverable, and there is no analytical service most buyers can practically use.
A higher price does not buy quality by itself either. What justifies one here is a batch-specific certificate from a named laboratory, running purity and identity methods, ideally with contamination screening, stating measured content rather than repeating the label. Compare against the same reasoning applied to NAD, where quantities are large enough that fill variance is easier to see.
