Why the NAD+ cost per vial is quoted per 100 mg
Run the per-milligram number and you get $0.052. It is arithmetically correct and completely useless.
A research peptide sells in 5 mg or 10 mg vials, so per-milligram is the natural unit there and the figures land somewhere between one and five dollars. NAD+ sells by the gram, so the same unit produces a number with two leading zeros that reads as free next to a peptide and tells you nothing. Per 100 mg keeps the figure in a range you can hold in your head and compare between listings, which is the only job a unit price has.

What is worth knowing is the structure, and one detail that explains why the structure looks the way it does.
Six hours is what the chair is for
The only published human study to infuse NAD+ directly, rather than a precursor, ran the drip for six hours. That is Grant and colleagues, writing in Frontiers in Aging Neuroscience in 2019, a pilot study tracking what happened to NAD+ and its metabolites in plasma and urine across the infusion. They reported no measurable change in plasma NAD+ or its metabolites until after the second hour.
Read as a purchase fact rather than a health claim, and it should be read that way, this is the reason clinic NAD+ is expensive in a way a vitamin drip is not. You are buying a long block of chair time, staffed. Several of the authors were affiliated with a wellness centre and an NAD+ research company, which is worth knowing when reading any conclusion drawn from it, and the study establishes nothing about benefit. It measured where infused NAD+ went.
The comparison, then, is not $52.00 against a session price. It is one payment for a gram of material against a recurring payment for supervised hours, and the two are not substitutes on any axis except the name on the invoice.

Ten percent off ten units does not come close to fifty percent off one. Whether the code sits on top of a tier is not something the vendor publishes, so assume it does not, budget on that basis, and check the subtotal in the cart rather than taking anyone's word for it.
The line that decides whether the price was worth anything
Price is the easiest thing to compare and the least informative, and on NAD+ specifically there is a documented reason why.
In a notice with content current as of 30 October 2024, FDA wrote that it was aware of compounders using food-grade nicotinamide adenine dinucleotide sold by repackagers to make intravenous products. Food-grade ingredients, it said, "are not suitable for compounding sterile drugs without appropriate processing, due to the high risk of contamination with microbes and endotoxins". The agency added that it had received adverse event reports following use of NAD+ injectable drugs, including severe chills, shaking, vomiting and fatigue, some requiring medical treatment, and that these reactions were consistent with excessive levels of endotoxins.
That converts directly into a purchase criterion, and it is the one this whole cluster turns on. Ask whether the material is pharmaceutical grade rather than food grade, and ask whether the certificate of analysis reports endotoxin testing, the bacterial endotoxins or LAL result, and not only identity and purity. A certificate showing HPLC and mass spectrometry has told you what the powder is. It has not told you whether it is clean, and clean is the thing that put people in a chair with rigors.
A cheap gram with no endotoxin line on its certificate is not a cheap gram. It is a different product at a different risk, priced as though it were the same one.
There is no insurance NAD+ price to compare against
Readers arriving from the prescription world reasonably ask what this costs with coverage. The question has no object.
There is no FDA-approved NAD+ injectable product, so there is no product code, no formulary line and no negotiated rate. NAD itself sits in Category 1 of FDA's 503A bulk drug substances list, which is the "under evaluation" category, and that is a compounding policy position rather than an approval.
