What FOXO4-DRI Is

A compound with a famous 2017 result, a clean mechanism, and nine years of nothing happening since.

FOXO4-DRI what is it, in one line: a laboratory-made peptide designed to make worn-out cells die on cue. It is a senolytic. It has existed since 2017, it works in mice, and in nine years it has not been given to a single human being in a registered trial.

The gap between rows five and six is the whole story. The mechanism got clearer, the animal literature grew, and the step into humans never happened.

FOXO4 and FOXO4-DRI are different things

The names differ by three characters and the things they name are unrelated in kind.

FOXO4 is a protein your body makes, a transcription factor in the FOXO family involved in stress resistance, metabolism and cell-cycle control. Ordinary human biology.

FOXO4-DRI is a synthetic peptide built to interfere with what FOXO4 does inside senescent cells. Your body does not produce it and it is not a supplement version of anything.

So searching FOXO4 versus FOXO4-DRI compares a human transcription factor against a designed tool meant to block it. Note the fourth character is the letter O, from Forkhead box protein O4; several vendors render it as a digit zero including in product URLs, which is a typo that propagated.

What D-retro-inverso means

DRI describes how the peptide is built, and it is the reason the molecule survives long enough to do anything.

Ordinary peptides are chains of L-amino acids, the form biology uses, which also makes them exactly what your proteases are built to digest. A D-retro-inverso peptide reverses two things at once: the amino acids are the mirror-image D form, and the sequence is assembled backwards. The result presents side chains in roughly the same spatial arrangement, so it still engages the same binding partner, while being far harder for enzymes to recognise and cleave.

One analytical consequence: because it contains the same atoms as its natural counterpart, it has the same mass. Mass spectrometry confirms composition but not stereochemistry.

How it works

Cells that accumulate damage can enter senescence, a permanent exit from the cell cycle. They stop dividing but do not die, remaining metabolically active and secreting inflammatory cytokines and proteases. They accumulate with age.

The puzzle the 2017 work solved was why they do not simply undergo apoptosis given their damage load. The answer was FOXO4: in senescent cells it binds p53 and holds it in a configuration preventing it from triggering cell death.

FOXO4-DRI is a decoy. It occupies the site where FOXO4 would bind p53, displacing the interaction. Freed, p53 relocates out of the nucleus and the cell dies, while healthy cells are largely unaffected because they are not relying on that interaction to survive. A 2025 paper identified the binding region as a disordered part of p53's transactivation domain.

Why the story stops there

Preclinical results failing to reproduce in humans is the ordinary outcome of drug development, not an unlucky exception. Laboratory mice have defined genetic backgrounds, controlled diets, controlled environments and short lifespans, and the endpoints chosen are ones measurable in that model rather than ones a person would recognise.

There is also a reason for caution that any human programme would need to address. Senescence suppresses tumour formation and assists wound healing, and a 2023 study found that clearing senescent cells could promote the development and progression of pulmonary hypertension. The mechanism cuts in more than one direction.
