Tirzepatide and Low Blood Sugar
Tirzepatide monotherapy in non-diabetic users causes hypoglycemia in roughly 4% of patients — mostly mild. The risk multiplies dramatically when combined with insulin or sulfonylureas, and that's where most serious events happen.
The Zepbound prescribing information reports hypoglycemia in approximately 4% of non-diabetic patients on tirzepatide monotherapy, with about 2% experiencing clinically significant (level 2) events at blood sugar below 54 mg/dL. Placebo rates are similar — about 1 to 2% — meaning the absolute risk increase from tirzepatide alone in people without diabetes is modest.
The risk profile changes substantially in people with type 2 diabetes, especially those taking insulin or sulfonylureas (glipizide, glimepiride, glyburide). In SURPASS-2 and other diabetes trials, hypoglycemia rates jumped into the double digits in combination users. The pharmacology explains the difference clearly, and so does the fix: dose reduction of the concurrent agent.
Why this happens
Tirzepatide's glucose-lowering effect comes through a glucose-dependent mechanism. It enhances insulin secretion when blood sugar is high and suppresses glucagon when blood sugar is high — but at normal or low blood sugar, those effects diminish. This is why tirzepatide alone rarely causes severe hypoglycemia in people without diabetes: the drug needs a glucose signal to drive insulin secretion, so it doesn't keep pushing insulin when blood sugar falls.
Insulin and sulfonylureas don't share that glucose-dependency. Once you've injected fast-acting insulin or taken a sulfonylurea, the drug keeps lowering blood sugar regardless of where blood sugar already is. Add tirzepatide on top, and you've stacked one effective glucose-lowering mechanism on another non-glucose-dependent one — and now you're vulnerable to hypoglycemia, especially if you skip a meal, exercise unexpectedly, or drink alcohol.
A second contributor is reduced food intake. Tirzepatide's appetite suppression means patients eat less and may skip meals entirely. People on basal insulin who were dosed for their pre-tirzepatide intake suddenly have proportionally too much insulin relative to their food. The same applies to sulfonylureas, which were calibrated to their old eating patterns.
A few medication interactions matter. Beta-blockers can mask the early warning signs of hypoglycemia (tremor, racing heart) without preventing the event itself. Alcohol acutely impairs the liver's ability to release glucose, which is why post-drinking hypoglycemia is a real and underappreciated risk on tirzepatide for some users.

How to manage it
Know the early symptoms. Classic hypoglycemia presents with shakiness, sweating, racing or pounding heart, hunger, lightheadedness, and a vague "off" feeling. Later or more severe symptoms include confusion, difficulty concentrating, slurred speech, vision changes, and eventually loss of consciousness or seizure. If you take a beta-blocker, you may lose the early adrenergic warning signs — rely on a meter and any unusual sensation.
Carry fast carbs. The standard 15-15 rule: if you feel symptoms or measure a fingerstick under 70 mg/dL, take 15 grams of fast-acting carbohydrate (4 oz juice, 3 to 4 glucose tablets, a tube of glucose gel, 8 oz regular soda), wait 15 minutes, and re-check. Repeat if still low. A package of glucose tabs in your pocket, bag, and car is cheap insurance. Pure sugar candy works in a pinch; chocolate doesn't (the fat slows absorption).
Get a meter — or better, a CGM. If you're on insulin or a sulfonylurea alongside tirzepatide, a continuous glucose monitor (Dexcom G7, FreeStyle Libre) is increasingly the standard of care. It catches the asymptomatic 3 a.m. lows, the post-exercise lows, and the gradual trends that lead to severe events. Insurance coverage has improved substantially.
Talk to your prescriber about reducing concurrent medications. Sulfonylureas in particular often need to be discontinued when tirzepatide is added — combining a non-glucose-dependent insulin secretagogue with a GIP/GLP-1 agonist mostly produces hypoglycemia without much added benefit. Basal insulin doses commonly get cut by 20 to 50% within the first three months, and rapid-acting insulin ratios get tightened.
Don't skip meals on insulin. Even if appetite is gone, eat enough to cover the basal insulin you took. A protein shake or a small balanced snack is better than nothing.
Be careful with alcohol. Alcohol on its own can cause delayed hypoglycemia 6 to 12 hours after drinking by suppressing liver glucose release. On tirzepatide plus insulin or a sulfonylurea, that risk compounds. If you're going to drink, eat alongside, check blood sugar before bed, and consider setting a CGM alarm.
