Week 9: Titrating to 7.5mg

Your first injection at 7.5mg is a 50% step up from 5mg. Expect a noticeable boost in appetite suppression and a short window where titration-style GI effects may return.

Abstract composition representing a step-change and adjustment

The Zepbound label lists 7.5mg as the second maintenance dose option after 5mg. The 50% jump is the largest relative increase in the titration schedule, so it is normal for the body to react more than it did at the 2.5mg-to-5mg step. Most users describe a "mini titration week" with somewhat sharper nausea, a quieter appetite, and a couple of days of low energy.

The good news: SURMOUNT-1 showed that higher doses of tirzepatide produced larger placebo-adjusted weight loss without a dramatic spike in serious side effects. Discontinuation rates for adverse events were low across the 5, 10, and 15 mg doses studied. The 7.5mg step is part of the path that gets most people to those bigger results.

The first week at a new dose often shows a slightly larger drop, partly from genuine fat loss and partly from reduced intake.

Nausea, diarrhea, and vomiting are the most commonly reported adverse reactions in tirzepatide trials, and rates rise modestly at higher doses. Eating small, frequent, protein-rich meals and avoiding very fatty foods reduces severity for most people.

The most important thing in week 9 is to under-eat carefully, not aggressively. A common mistake is letting intake drop below 1,000 calories because food simply does not appeal. Aim for at least 80–100 grams of protein and steady hydration — this protects muscle, energy, and mood through the adjustment.

What Users Report

The first 48 hours on 7.5 were rough — back to the early-titration nausea. By day 4, it was honestly the best appetite control I had felt since starting.

— Hannah L.

Dropped four pounds the first week at 7.5mg. I had to actively remind myself to eat, which is a strange place to be after years of the opposite problem.

— Diego F.

Looking ahead

In week 10 the body adapts to 7.5mg and the second-week pattern becomes the new normal. Read Week 10 on Tirzepatide for what to expect, or browse more resources.
